The acute response is generally self-limiting and is associated with increased bronchial and pulmonary hyperreactivity to contractile agonists and enhanced bronchial responsiveness to mediators, but is followed by an accelerated phase of recovery. The continued presence of inflammatory cells in the lung, however, leads to formation of in situ granulation tissue. The granulation tissue is formed by fibroblasts which in response to mediators produced by the inflammatory cells, produce collagen and other extracellular matrix components and express increased numbers of contractile proteins, including myofibroblasts. The fibroblasts are contractile, and in addition to depositing extracellular matrix, contract granulation tissue and other tissues, they also exert a profound influence on the respiratory tract structure and function. It is this process of contraction and remodeling which leads to the long term consequences of scar formation, resulting in narrowing of the airways and loss of normal elastic recoil of the lung.
